Employers, Hospitals, and other non-individual users:
Please contact a Copay representative for information on group and custom pricing plans.

Individuals:
To process your enrollment, you will need to fill out a series of forms, which should take no more than 5 to 10 minutes. Click HERE to download forms.

When you have completed the enrollment forms, you will need to sign them to authorize us to advocate on your behalf vis à vis insurance companies, doctors and hospitals. Please print out the forms, sign them, and mail or fax them back to us. After receiving your enrollment forms, a Copay Solutions representative will contact you shortly to assist you in getting started.

PLEASE NOTE that we are currently refining our secure payment and online viewing functionality - these features will be available shortly. All core features (bill audit and coordination with insurers and providers, etc.) are of course fully functional.
